The Transition Services Program is designed to provide individuals with developmental disabilities with financial support on a one-time basis while they undergo transitions in living arrangements/service providers. This program enables individuals to make arrangements and obtain those items necessary to set up their next household with the exception of room and board. Services under this program are provided to those individuals who are transitioning to a provider setting or a private living arrangement from an institution or another provider setting. As individuals eligible for this program are moving to a living arrangement where they will be responsible for their own living expenses, the desired outcome of transition services under IHCOS is to allow the individual to set up their new household to ensure a smooth transition and minimize challenges. |
